Records Request

• Complete and submit the Record Request (RR) Form (English) OR Record Request (RR) Form (Spanish) along with proper identification; see Record Request Form instruction page for a list of acceptable forms of identification
• Provide an email address if the delivery option email is chosen
• Provide a fax number if delivery option fax is chosen
• Provide mailing address if delivery option mail is chosen

Appropriate medical expertise is required for the correct interpretation of clinical laboratory results and is not available from laboratory personnel. Action should not be taken based on test results without first discussing them with your healthcare provider.
